NDIS Support Coordination City of Armadale
Brightwest Care’s support coordinators are dedicated to improving participants’ skills in coordinating their supports. Our approach involves working hand-in-hand with participants to gain insight into their funding, clarify their expectations, and understand their preferences for support design. We ensure that a strategic combination of supports is set up to enhance each participant’s capacity and independence. Our efforts include maintaining crucial relationships, managing service delivery tasks, and fostering social and community inclusion. We strive to provide the guidance needed for participants to manage their supports effectively and achieve their personal goals.
NDIS Recovery Coach
This support item helps participants to build capacity through strong relationships, to support people with psychosocial disability to live the life they want. This support is designed to increase social participation and economic independence.
Recovery coaches work closely with participants, their families, support workers and other services to:
- Help you get the most from your NDIS Plan
- Assist with coordination of NDIS supports
- Implement a recovery plan to help you become more independent
- Connect you with mainstream supports.
- Provide coaching to increase skills and capacity such as motivation, strengths, resilience and decision-making.
NDIS Support Connection
This support item assists you to connect with the broader system of support; informal, community and funded supports. It enables you to understand your NDIS plan and achieve your goals.
